This is required to verify that you were given this information at least 24 hours before the procedure. The State of Ohio does not have these materials online or in written form as of the effective date of the bill. Consent Form #2: A consent form which certifies that you are terminating your pregnancy freely and without coercion. It details the possible risks and complications associated with the abortion procedure and anesthesia type; the gestational age of the fetus documented by ultrasound; the name of the physician/facility where the termination will take place and all other associated risks of a voluntary/elective surgical procedure. You will receive this form at the Cleveland Women’s Medical Group the day of your appointment.
